Can you please check if Milfy_City.exe rightclick->Properities->Compatibility: Are the options "Override High DPI scaling behaviour.." and the "Disable fullscreen optimizations" are unchecked? Basically every settings there are unchecked?
Your full screen issue is probably related to resolution or scaling error. Try to right click > properties of the .exe file and check "Override high DPI scaling behavior" or "Disable fullscreen optimizations." Or uncheck them if active.

The girls basically vanish from the game after you acquire the second heart with them minus the night time sleep.
For Caroline, you need to talk with her for the second heart after you made the lats photoshoot with her (the one in the full fishnet when she allows you to hotdog her) then you have to talk with her at home in her room about sales, then go to sleep while using the bed instead of Linda/Mom.
